What is an advance health care directive?

An advance care directive is a formal way of giving instructions on what medical treatment you want or do not want if you are not able to communicate because of illness or injury. It is sometimes called a ‘Living Will’.

DWDnsw suggests you complete one and provides all new members with two alternative forms.  Subsequent forms can be purchased from us or downloaded for free – see below.

 Should you appoint someone to make decisions on your behalf?

DWDnsw also suggests that you appoint someone you trust to act as your enduring guardian/s. An enduring guardian has the legal power to make health care decisions on your behalf. DWDnsw provides all new members with a form that can be used to appoint an enduring guardian. Subsequent forms can be purchased from us or downloaded for free – see below.

Remember — every competent adult has the legal right to accept or refuse any recommended health care.
Please note that completing an ACD does not enable you to access treatments which are currently deemed to be against the law of NSW, such as medically assisted dying. In addition, an ACD cannot compel a doctor to administer a particular treatment.
